Pathways Community Services joined Live Well San Diego as a Recognized Partner on July 18, 2016. Pathways Community Services specializes in integrated behavioural health services in community and home settings. Pathways Community Services has serviced thousands of children, youth, adults, and families in cutting-edge programs that are marked by an unwavering commitment to exceptional value and recovery-focused care.
Pathways Community Services offers accessible, integrated and culturally competent community-based care. Their recovery and wellness programs instil hope, responsibility, empowerment and meaningful roles. Their evidenced-based practices and outcome driven services have demonstrated reduced psychiatric and medical hospitalizations and improved quality of life.
Pathways Community Services’ teams include therapists, case managers, nurses, psychiatrists, substance abuse specialists, housing specialists, education/employment specialists, and peer support specialists, among others. They engage in collaborative partnerships with a wide array of providers in the community, including health care and other behavioral health services, inpatient and outpatient services, and in facility-based and community-based care settings.
