Health care centers across the nation are experiencing a growing demand in the number of patients over the age of 55 that are looking to access medical, dental, mental health and support services. This aging population of patients comes with a special set of health care and support needs that intertwine health services with social services. To meet this growing demand and the population’s specific set of needs, Family Health Centers of San Diego (FHCSD) launched a new Senior Services program at their Downtown Connections clinic.
The Senior Services program provides comprehensive, wrap-around services that address barriers to care for individuals ages 55 and above. The new program is focused on ensuring individuals have access to primary care services, social work and case management, benefits and entitlements counseling, mental health services, physical therapy and more. The program also offers information and referrals to outside agencies that offer services to this demographic.
FHCSD has provided affordable, high-quality health care and support services to San Diego residents for more than four decades, with a special commitment to serve diverse populations who are uninsured, low-income and medically underserved. This holistic approach is one way FHCSD is working to improve seniors’ overall health and wellbeing, as well as meet their individualized needs.
