Aging

Partners involved in the Age Well San Diego initiative are working toward making our communities more age-friendly for people of all ages to live healthy, safe, and thrive.

The Aging Roadmap is a regional plan developed by a wide range of community partners that support older adults.   

Health Equity

The Healthy Cities, Healthy Residents project seeks to create healthy communities by supporting community-based organizations, residents, and cities in working together to advance healthy and equitable planning, policies, and neighborhood environments.

Mental Health

San Diego Trauma-Informed Guide Team involves multiple partners to promote the development and provision of trauma-informed services in San Diego County’s agencies and systems through collaboration, advocacy and education to make significant strides to address childhood trauma.

Nutrition & Physical Activity

The San Diego County Childhood Obesity Initiative, facilitated by Community Health Improvement Partners, has seven workgroup domains that engage partners across sectors to help shape obesity prevention.

Prevention and Access

Palomar Health Community Action Councils identify and address the healthcare needs of communities through mutual support and partnership.

Substance Abuse

The San Diego Prescription Drug Abuse Task Force is a Countywide initiative comprised of key stakeholders,  community members and local experts working together to decrease the harms associated with the misuse and abuse of prescription drugs in San Diego County.

Gang Commissions

North County Gang Commission coordinated by the  County of San Diego and San Diego Gang Commission coordinated by  City of San Diego work to develop a more strategic and coordinated effort between cities, law enforcement agencies, social service providers, and the general public with the objective of curtailing gang involvement to bring about a change for our region.

Homeless

The Regional Task Force on the Homeless is an integrated array of stakeholders committed to preventing and alleviating homelessness in San Diego which provides essential data and insights on the issue of homelessness, informing policy and driving system design and performance.

Military/Veterans

The San Diego Military Family Collaborative Council and Action Teams, facilitated by SAY San Diego, are comprised of more than 400 representatives from over 100 unique public, private, faith-based, military, and governmental organizations and work to maximize the collective impact of community resources to enhance military family life.

Older Adults/Disabilities

NorCAN, ECAN, SanDiCAN and SoCAN are action networks that advocate for and improve the lives of older adults, persons with disabilities, and their families by working efficiently and collaboratively to enhance the services they receive.

Workforce

Coordinated by South Bay Community Services, SDPromise brings together a collaboration of partners including schools, local government agencies, non-profit organizations, health services and businesses to develop a plan for a seamless continuum of cradle to career solutions that creates high expectations for all children and provide a clear and viable pathway to college and career success.

Youth

An initiative of the San Diego Workforce Partnership, OpportunitySD is a partnership to tackle youth disconnection from school and work.

Active Transportation

SANDAG’s Active Transportation Working Group makes recommendations and fosters cooperation among jurisdictions, agencies and stakeholders to plan for and support the development of local and regional improvements for active transportation modes, including Safe Routes to Transit and Schools.

Climate

The San Diego Regional Climate Collaborative was established in 2011 as a network for public agencies to advance climate change solutions that mitigate greenhouse gas emissions and adapt to the effects of climate change. Partnering with academia, non-profit organizations, and business and community leaders, the Collaborative raises the profile of regional leadership, shares expertise, and leverages resources.

Food

San Diego Hunger Coalition leads many strategic partners in coordinated action to end hunger in San Diego County supported by research, education, and advocacy so that everyone in San Diego County has enough food for an active, healthy life.

Increasing Energy Efficiency

Many partners participate in San Diego Regional Energy Partnership, San Diego Association of Governments (SANDAG) Energy Working Group, San Diego Regional Clean Cities Coalition, and other local partnerships to increase energy efficiency and reduce energy consumption.

Regional Community Leadership Teams

Regional Community Leadership Teams connect the collective efforts of organizations and individuals and facilitate opportunities for stakeholders to have a voice in the future of their neighborhoods, so that every resident can be healthy, live safely and thrive.

                       

 

SIGNATURE EVENTS

 

Each of the Live Well San Diego Signature Events listed below bring 1,000’s of people together in support of healthy, safe and thriving communities. Participation and support from Recognized Partners in these events strengthens partner-to-partner collaboration and involvement in the vision.

Partners have the opportunity to participate as planners, speakers, sponsors, hosts, volunteers or participants. At the most basic level, partners can help with promotion of Signature Events – when we work together to share content we can greatly expand the reach of programs to get San Diego County residents involved and living well!

 

Check Your Mood

Check Your Mood is an annual event held in conjunction with National Depression Screening Day held on the second Tuesday of October. The goal of Check Your Mood Day is to engage and encourage San Diegans to monitor and assess their emotional well-being just as they might check their blood pressure or be screened for heart disease or diabetes. Organizations across the county provide free mental health resources, information and Check Your Mood screenings to the community to help raise awareness for mental health.

Communications Summit

The Communications Summit, held in Spring, brings together communications professionals from Recognized Partner organizations to network and learn ways to better collaborate and align messaging surrounding the Live Well San Diego vision.

Data Summit

The Data Summit, held in Summer, helps attendees discover new data resources, publicly available data and data visualization tools to help organizations showcase their value and impact.

Live Well Advance

The Live Well Advance is the Live Well San Diego partner conference and brings together thousands of partners and stakeholders to network, learn about new tools and best practices, and participate in breakout sessions each Fall. Leaders from every sector are invited to advance our shared vision of a healthy, safe and thriving San Diego region.

Live Well San Diego 5K & Fitness Challenge

The Live Well San Diego 5K and Kids' 1-Mile Fun Run is held on the last Sunday in July and brings individuals, families and organizations together in support of healthy, safe and thriving communities. Live Well San Diego 5K Fitness Challenge runs from July 1 to 31st to help participants increase their physical activity and prepare to run or walk in a 5K. Participants receive a daily email or text offering a fitness challenge and training tip from partners.

Love Your Heart

Love Your Heart is an annual event in which organizations from across the U.S. and Mexico join together to provide FREE blood pressure screenings to the public on Valentine's Day, February 14. The event helps thousands of participants to get to know their blood pressure numbers and take charge of their own heart health.
