Community Health Improvement Partners (CHIP) joined Live Well San Diego as a recognized partner on June 24, 2014. CHIP is a collaboration of San Diego health care systems, hospitals, community clinics, insurers, physicians, universities, community based organizations and the County of San Diego who are dedicated to a common mission: to assess and address priority health needs. CHIP became a Live Well San Diego partner by establishing a model of collaboration based on collective impact, by bringing together diverse partners to develop a common understanding of complex community health problems and their root causes, and by championing key initiatives to solve San Diego County’s most complex health problems. The Resident Leadership Academy, Independent Living Association, the Needs Assessment and the Suicide Prevention Council are examples of collaborative initiatives lead by CHIP to address critical needs in County of San Diego.
CHIP is also paving the way in health and wellness programs. Its San Diego County Childhood Obesity Initiative (COI) works to reduce and prevent childhood obesity through policy, systems, and environmental change. Modeled after the COI, the Lemon Grove Healthy Eating Active Living (HEAL) Zone is funded by Kaiser Permanente as a collaborative of local Lemon Grove residents and organizational representatives working to build a city that is more active and health-conscious. As a recognized Live Well San Diegopartner, CHIP will continue to put the health and well-being of San Diego communities at the forefront of its mission.
