Vista Community Clinic (VCC) joined Live Well San Diego as a recognized partner on October 30, 2014. VCC became a Live Well San Diego partner by positively impacting the health, safety, and wellbeing of the community through its healthcare services and community health programs. Recognized as a key regional health provider since 1972 with five state-of-the-art clinics in Vista and Oceanside, VCC treats almost 60,000 patients each year through an innovative model of community health. That model provides low-cost, high-quality health care to all who walk through their doors.
VCC’s premier health services include primary care for adults and children, women’s healthcare and dental services. As a Level 3 Patient Centered Medical Home, a distinguished accreditation granted through the National Committee for Quality Assurance, VCC has achieved the highest standards in patient care. This team-based approach emphasizes communication, quality and access.
Not only does VCC provide quality healthcare, but their Health Promotion Center, one of the largest in San Diego County, offers innovative and effective education, outreach and advocacy programs geared to help the community “choose health”. VCC also recognizes the importance of collaborating with other agencies to achieve their mission and vision. As such, they build and sustain relationships that promote community health and wellness. VCC has been an important member of the North County Community Leadership Team, a group of community stakeholders who provide leadership and direction for addressing health, safety, and wellness issues specific to North San Diego County. VCC supports the Live Well San Diego initiative and is committed to continuing efforts to promote programs and policy changes that help residents make positive choices for health and improve their lives.
