Family Health Centers of San Diego

Family Health Centers of San Diego (FHCSD) joined Live Well San Diego as a recognized partner on July 17, 2015. A nonprofit network of health centers founded in 1970, FHCSD is the largest community clinic provider of health care to the uninsured in the county. Since its establishment, FHCSD has expanded from a single community clinic in the Barrio Logan neighborhood to over 35 locations throughout San Diego County. In addition to primary care sites, dental clinics and behavioral health facilities, FHCSD operates three fully functional mobile clinics that visit community sites such as schools, parks and social service agencies.

FHCSD supports Live Well San Diego’s vision by actively promoting positive and healthy choices. FHCSD hosts health fairs and workshops that provide health screenings as well as nutrition, physical activity and mental health education to the community. FHCSD also offers specialized services for different sectors of the community with programs that focus on prenatal care, pediatrics and chronic disease management. Several FHCSD programs have received recognition for their innovative approach, including:

  • Safe Point San Diego for removing over 2.5 million used syringes from the streets
  • The Cholesterol and Diabetes Comorbidities Management Program, which the U.S. Department of Health and Human Services recently recognized for achieving hypertension control rates of over 70% among patients
  • The Baby Boutique, which has encouraged more than 14,000 women to obtain pre- and postnatal care.

FHCSD is committed to its mission of making affordable, high-quality health care and supportive services accessible to everyone, especially uninsured, low-income and medically underserved persons. Partnering with Live Well San Diego is an additional way FHCSD is able to provide essential service throughout San Diego County.
